For New Patients

We value your time, so that your visit is meaningful we would ask that you do the following at least 48 hours or more prior to your appointment. Please give us 48-hour notice if you are unable to keep your appointment.

Download the New Patient Packet Form and fill it up.

Email Us

Send us your filled-up new patient packet form and a copy of your insurance card (front and back) via email at contact@sarasotaFCendo.com .

Bring Your Documents

Please call your primary and/or referring doctor and have your records faxed to us at 941-259-0218.

    • Last Doctors office/consult note

    • Last 2 years of lab results

    • Any imaging or studies (US thyroid, CT, MRI) as applicable for your visit

    • Any Thyroid biopsy results

    • Any Surgical pathology reports ( ie thyroid surgery, pituitary surgery)

Our Fees and Policies

FEE FOR SERVICE

We are out-of-network with all insurances and payment is due at the time of service.

We will provide you with a superbill that you can submit to your insurance.

Privacy Policy

For Follow-up Patients

  • You will get an Appointment Reminder Text Message 1 week prior to your appointment. Please confirm by replying 1 via text.

  • Do your labs 1 week prior to your appointment

    Your lab orders will be sent electronically at your last appointment with Dr. Desai to your preferred lab ( ie. Labcorp or Quest).

For Diabetic Patients - New or Follow-up Appointment

  • Download the continuous glucose monitoring (CGM)  app on your smart phone ( ie. Freestyle Libre or Dexcom) and link your account to our practice (ID: fullcircle).

  • Create an insulin pump account ( Medtronic carelink, Tslim) and link to our practice (ID: fullcircle).
